Student or General Public Injury & Property Damage

Any accidents or incidents which result from, arise out of and directly relate to the University’s premises, operations or an employee’s position of employment must be reported in writing.

Accidents & Incidents

The procedures outlined in this section pertain to accidents and incidents except vehicle accidents [refer to 8:040 Vehicle Accidents for information on vehicle accidents]. This section applies to:

  • Any occurrence which results in bodily injury or damage to property of a student or member of the general public.
  • Any occurrence that results in a threat or statement of intent to take legal action against the University or an employee because of an alleged personal injury (libel, slander, false arrest, etc.).

Reporting an Accident or Incident

Report an accident or incident in writing to the Division of Finance, 311 Jesse Hall, within 48 hours after the occurrence.

Required Form

The Student or General Public Injury and Property Damage Report (UM 200) is to be initiated by:

  • The faculty member in charge of the student’s activities at the time of the accident or incident or to whom the accident or incident was reported.
  • The person in charge of the building or facility or the person sponsoring the meeting or even attended by the student or member of the general public at the time of the accident or incident.
  • Any employee who witnesses an accident or who receives a threat or statement of intent to take legal action for personal injury is made.

Preventing a Similar Accident

The person who initiates the report should try to determine whether the accident was caused by an unsafe condition; and, if so, take the action necessary to prevent the occurrence of a similar accident, such as reporting the accident to the appropriate office (e.g., Campus Facilities, Environmental Health & Safety).
