Damage to University Property
Adopted: April 1998
Responsible Office: UM Risk and Insurance Management
Any damage to or destruction of University property, including fire, must be reported to Risk and Insurance Management within 24 hours after discovery of the occurrence.
This policy is in compliance with UM Policy 13201 – Report All Accidents & Incidents.
Written Damage Report Required
The department or the individual who discovered the fire, damage, or destruction must initiate and submit a Damage Report (UM69) to Risk and Insurance Management via email at firstname.lastname@example.org.
Within 24 Hours
The report must be submitted to Risk and Insurance Management within 24 hours after discovery of damage to:
- Any University-owned or leased building or facility.
- Any University equipment, machinery, furniture or fixtures which are essential to the operations or use of the building or facility.
NOTE: This does not include any tools, supplies or other expendable items.
